How Does Refractive Surgery Work?

Refractive errors occur when the cornea is misshapen so that light doesn’t refract properly onto the retina. Refractive surgery uses a precise laser to reshape the cornea to ensure the light lands directly on the retina to create a clear image.

  • Those who are nearsighted will need their cornea flattened
  • Those who are farsighted will need their cornea steepened
  • Those with astigmatism will need their cornea smoothed

What Types of Refractive Surgery Are There?

Laser eye surgery has come a long way since its beginnings in the 1980s, with new advancements allowing for faster healing, fewer side effects, and less postoperative pain. The 2 most commonly performed procedures are LASIK and PRK, but several other refractive surgeries are also performed.

LASIK, or laser-assisted in situ keratomileusis, involves using a blade or laser to cut a flap in the cornea’s outer layer, called the epithelium. A computer-guided laser is then used to reshape the corneal tissue underneath, and the flap is replaced to act as a natural bandage.

LASIK involves minimal recovery time, typically a few days, with almost immediate vision improvement.

PRK, or photorefractive keratectomy, involves removing the outer layer of the cornea completely to reveal the corneal tissue underneath. A laser is then used to reshape the tissue beneath, and a contact lens bandage is placed overtop to promote healing.

PRK is generally more painful than LASIK, with week-long recovery time and vision improvement in around 2 weeks.
